Job Summary

At 85 Hope, we believe in providing quality healthcare to those in need, and we're seeking a skilled professional to join our team on a part-time basis working 2 afternoon/evenings per month. The Nurse Practitioner will play a crucial role in health coaching and guiding patients through their medical journeys.
Duties
- Conduct comprehensive patient assessments to determine health status and needs.
- Develop and implement individualized care plans for patients.
- Provide health coaching to empower patients in managing their health conditions.
- Collaborate with multidisciplinary teams to ensure coordinated care for patients.
Qualifications
- Valid Nurse Practitioner license and certification in the state of practice.
- Proficient in patient assessment techniques and case management strategies.
- Strong communication skills for effective health coaching and patient education.
- Knowledge of medical practices and procedures relevant to the role.
- Ability to work collaboratively within a healthcare team while maintaining a patient-centered approach.
